The University has a participant accident protection program designed to provide group accidental medical insurance to all participants while involved in UNT youth programs. Program managers must purchase accidental medical insurance for the group through Risk Management Services (RMS) if it cannot be verified that all youth have personal medical coverage.  If a program manager confirms all participants have medical insurance, they can still purchase this as a secondary coverage.

Please complete the Youth Program Insurance Request form to submit your request. 

REMEMBER: If you cannot certify all participants have personal medical coverage, you are required to purchase program insurance.
